STEP 1: REGISTER

I will review the information you have provided and get back to you within 48 hours. Expect email and/or text with a link to the patient portal.

Step 2: COMPLETE INTAKE

Follow the prompts, read, sign. If you prefer paper, you have this option as well – follow this link to the Forms page. You will need to print out documents ##2, 3, 4, 5, 6, fill them out, and somehow (email?) get them to me. Keep in mind that going the paper route may slow things down.

 

 

STEP 3: BOOK FIRST APPOINTMENT

Now that you are registered, select the date and time, and you are all set.

Patients are encouraged to make their own appointments. This helps me better understand your health care needs and avoid any future misunderstandings. One exception: a house call to a patient with prohibitive physical attributes.
