Patient Rights & Responsibilities
Point of Cure Acupuncture and Electromedicine (POC) is committed to providing quality health care. In accordance with the Florida Statute 381.026., we present the following Patient Rights and Responsibilities:
As a patient, you have a right to:

Receive competent and respectful health care.

Have equal access to services, regardless of race, creed, national origin, religion, age, disability, other protected category, or source of payment.

Receive care taking into consideration your spiritual and cultural beliefs, personal and ethical values and educational needs.

Have your symptoms taken seriously.
-
Receive adequate assessment and treatment of pain.
-
Be treated with respect, consideration, and dignity.
-
Expect a safe environment free of neglect, exploitation, or abuse.

Be seen in a timely manner.

Have your family and/or support individual(s) involved in care, treatment, and services when appropriate.

Select another person to make health care decisions in the event you are unable to do so.

Know the name, role and qualifications of your health care provider.

Know what patient support services are available, including language interpretation and translation for non-English speakers.

Receive adequate information necessary to make an informed decision about any treatment or procedure, except as limited in an emergency.

Give or withhold informed consent before starting a procedure or treatment.

Refuse treatment and to be advised of the alternatives and consequences of your decisions.

Withhold consent to have your images, films or other recordings used for purposes other than your care.

Have your health care information and all communications kept confidential.

Review your medical record and approve or refuse (with some limitations) the release or disclosure of its contents to any health care practitioner and/or facility.

Request that your provider not make notes in the chart pertaining to sensitive matters.

Receive complete, current and easy to understand information regarding your diagnosis, treatment options and, and follow-up care.

Participate actively in decisions about your healthcare.

Receive a prompt and reasonable response to questions and requests.

Know what medications or treatments are prescribed, why, expected outcome, and side effects.

Be informed of unexpected outcomes related to care.

Know, in advance of treatment, whether Medicare assignment is accepted.

Receive on request a reasonable estimate of charges for medical care (prior to treatment).

Receive a reasonably clear and understandable itemized bill (after treatment) and have the charges explained.

Obtain a second medical opinion from the clinician of your choice, at your expense.

Receive information about the methods available to file a complaint or grievance regarding your care and know that you will not be retaliated against for filing such grievance.

Know what rules and regulations apply to your conduct.

Receive a copy of this Bill of Rights at any time.

As a patient, you have the responsibility to:

Provide correct and complete name, address, telephone and emergency contact information.

Provide us with current and complete insurance information, including any secondary insurance, each time you see us.

Provide complete and accurate information concerning your health including current and past illnesses, diagnostic tests, health care providers and treatments.

Sign a “Release of Information” form so we can obtain your old medical records.

Tell us about all prescription medications, supplements, or over-the-counter medications you take.

Timely report changes in your condition or reactions to medications or treatment

Ask your clinician questions about your illness, treatment plan or medication instructions.

Follow the treatment plan that was agreed on and accept responsibility if you refuse treatment or do not follow our instructions.

Comply with referrals to specialists and diagnostic facilities.

Keep follow-up appointments and notify us if you are unable to keep them.

Meet your financial obligations promptly.

Follow our rules and regulations affecting patient care and conduct.

Treat the staff and other patients in a courteous and respectful manner.

Advise us immediately of any dissatisfaction you have regarding your care.
