Action Plan - Verify Insurance
STEP 3: VERIFY INSURANCE
Please refer to these instructions as you fill out the form
Check your email. You should see an email from Salud Medical and Consulting. Open it. Click the blue “Complete Intake” button in the body of the email.
Screen 1: Landing page
Once you have landed on the Carepatron’s website, you will be greeted with the following message: “Salud Medical and Consulting LLC has invited you to join their Carepatron platform.” Don’t get distracted – you are not really “joining” anything at this point. If you become an active patient, you will create an account at that time. But for now, I am simply using this HIPAA-compliant platform to securely exchange information with my prospective patients. So go ahead and click on the “Sign up" button.
Screen 2: Personal details
Basic stuff we need to verify your insurance coverage. Fill it out. Click the “Next” button.
Screen 3: Documentation
I must have the images of the front and back of your insurance card. Attach (or drag and drop) each picture at a time. When you hit the “Next” button, the images will be uploaded to the server. If you are stuck, don’t worry. You will text both pictures to 321-216-9000 after completing the rest of the intake. Click the “Next” button!
Screen 4: Forms and Agreements
You should see one form listed: Insurance Verification. Click the blue “Fill out” button.
Screen 5: Insurance Verification
This step is crucial! Fill out as accurately as possible. Sign the form. Wait several seconds until the message underneath the signature turns to “Signed by …”. Scroll all the way up and hit the blue “Submit” button in the upper right corner.
Screen 6: Forms and Agreements (again)
You are taken back to the “Forms and Agreements” page. Click the “Next” button.
Screen 7: Payment Method
Ignore it. At this point, no payment is required on your part (I am paying the billing company to verify your insurance). Unclick “Allow Salud Medical and Consulting LLC to use the saved card in the future” and simply hit “Finish.”
Screen 8: Sign in to your workspace
Same thing. Ignore. You are done. Exit the webpage.

If you are stuck, no worries. You can always default to pencil and paper. Please download this PDF form, print, and fill it out completely. Don’t forget to attach both sides of the insurance card.

The information you have provided will be forwarded to an outside billing company. Please give them a couple of days to verify your benefits. I expect to get back to you with an estimate of your costs in about 24 to 48 hours.
